Dear MESD Parents,

We have partnered with the Phillips County Health Department to provide our students that are 5-11 years of age  with the Covid-19 vaccination.  The clinic will be held in the Elementary library on December 2nd from 9:30a.m. until the last student has received their shot.  In order to receive the shot, the parent must fill out, sign, and return the attached form by November 29th.  We look forward to taking the next step in keeping our students safe from this virus.
