Sport physicals will be on August 9th at 9:00a.m. at the School Base Health Center (SBHC). To have a physical done the athlete will need the consent form from the SBHC filled out as well as the Health History form filled out and signed by the parent of the athlete. I need both forms on the 9th. The Athletic Director, Dr. Laurent has the packet with these documents. If you have any questions please contact me or your AD.
